If you would like to apply for an account, credit terms or to authorize us to charge all future orders on your credit card, please select the applicable forms from the list below. Once you have completed them, please fax, email or mail the forms to us.

Account Application Form: This form is for new accounts that want to pay by credit card or COD, and are not interested in net 30 day terms. If you pay by credit card, please also fill out the Credit Card Authorization Form. It will take 1 business day to set up an account. Click here. to download the form.

Credit Card Authorization Form: This form is to authorize Unique Wholesale Jewelry to charge your credit card on all future shipments until notified in writing. Click here to download the form.

Application For Credit Form: This form is for new accounts that would like to apply for Net 30 Day terms. This application can take up to 30 days, based on the response from vendors listed on your application. Click here to download the form.
